If you want to stay in your own home as you get older, but need help with a range of services, a Home Care Package may be for you as it provides a coordinated range of services (eg personal care, support services, clinical care) tailored to meet your specific care needs.
Home Care Packages are funded by the Australian Government through the Home Care Packages Program. There are four levels (1-4) of Home Care Packages which are designed to give you the care you need now, but can also be changed if your needs increase later.
Funding associated with packages is allocated to you directly – giving you the flexibility and freedom to choose your preferred service provider and change at any point if you are unhappy.
What is Consumer Directed Care?
All Home Care Packages will be delivered on a Consumer Directed Care (CDC) basis. CDC gives you more control and choice about the types of care and services you access, how the care is delivered and who delivers it to you.
Under CDC, you will determine the level of involvement you would like to have in managing your own package. You will be provided with a personalised budget so you can see how much funding is available for services and how the money is being spent.
How Do I Get Support?
To access support as you age you will need to call My Aged Care on 1800 200 422. They will then ask you a number of questions to determine your eligibility and create a Client eRecord. If you need an interpreter, all you need to do is phone on 13 14 50 beforehand and let them know what language you speak.
Before you can be assigned a package, you will need to be assessed as eligible by an Aged Care Assessment Team (ACAT). The assessment team is made up of a range of health professionals to help you work out what your care needs are, and whether a Home Care Package would assist you.
Once you’ve been assessed, you will be placed on a National Package Queue. Your position in the queue will be based on your individual needs and how long you have been waiting for services. Once you have received the top of the queue, you will receive a letter informing you that your package is ready. You then have 56-days in which to select your service provider and commence services.
